Before your appointment

• Schedule brief 15 minute phone call to discuss model of care care, address any questions and set up consultation time

• Currently I only have capacity for consultations, I will not be able to provide ongoing care

• I am currently out of network for ALL insurance companies

During your appointment

• Appointments are around 60-90 minutes

• We will complete a psychiatric history and evaluation

• We will discuss recommendations and a shared decision making approach to guidelines

• Billing will be completed

• I do not accept insurance at the moment

• Some insurance companies may provide reimbursement for care received with a super bill provided

After your appointment

• A write up of your evaluation and recommendations will be provided within 7-10 days to take to your prescribing psychiatrist, primary care physician or OBGYN
